For current Lahainaluna students only!

Parent Permissions Form for Transcripts   

(For students under 18 yrs. old)

Students may freely request transcripts once the Parent Permission Form is signed and filed with Miss Kristy Arakawa in the College & Career Center.

The first 2 transcripts requested by any high school student is FREE. Every transcript after the first 2 will cost $1.00 per transcript. Students will be billed at the end of the school year.

Please download the PDF and have a parent sign the form. You may deliver the signed form to Miss Kristy Arakawa in the College & Career Center OR email the signed form to her. [kristy.arakawa@k12.hi.us]
