Transcript Request Form

For current Lahainaluna students only!

Important Info

You must request your transcripts at least 2 weeks before the deadline.

  • A complete address must be given, only if Miss Kristy must MAIL the transcript.

    • If not, only the name of the college or scholarship is needed​

  • Miss Kristy can only email unofficial transcripts to parents/students.

  • If Miss Kristy must email a transcript to a college or scholarship, please give her the email address.

  • If you need help completing the request form, please see Miss Kristy for assistance.

Students may freely request transcripts once the Parent Permission Form is signed and filed with Miss Kristy Arakawa in the College & Career Center.

The first 2 transcripts requested by any high school student is FREE. Every transcript after the first 2 will cost $1.00 per transcript. Students will be billed at the end of the school year. This has been waived for the 2023-2024 School Year, all transcript are FREE this year.
